The circuit in the figure is based on two integrated LM108 and its gain is basically determined by the relationship between R1 and R2. The input impedance is several megohms, and the resistors used, as in all circuits of this type, must have a very narrow tolerance. This tolerance, which determines the matching of the components, influences the rejection in common mode. Capacitor C1 must be obtained experimentally to maximize the through range.
